FORMS THAT STUDENTS & ADVISORS SHOULD HAVE

Event Request Form:
Use form to outline the details of your event, who is involved, and to obtain the necessary permissions from your Faculty and Kiwanis Advisors before holding the event. This form should be filled out and given to the advisor(s) 
for every service event or fundraiser.

Medical Authorization and Consent to Attend Form:
Two copies for the year per student should be sufficient: one for the student to keep on them at all events and one for the advisor to keep. Advisors should have a binder with these for all members that chaperones have on hand at all events. Each year's binder should be kept for three years, then destroyed.
Photographic Release Form:
Two copies for the year per student should be sufficient: one for the student to keep on them at all events and one for the advisor to keep. This form is signed by parents and guardians as a safeguard who do not wish their child to be photographed at a Kiwanis or Key Club event which may be used for promotional purposes.

RULES & REGULATIONS

"Chaperones who accompany youth to overnight events or are regularly-used chaperones must be background checked. Chaperones that are chosen to accompany students to large-scale or long-term events should be willingly active in participating and supervising the youth and should be physically fit and/or have the means to be able to handle student emergencies quickly. Chaperones should also have student medical forms and contact information easily accessible at all times.

At typical NON-overnight Key Club events, one chaperone per twelve mixed-sex students is acceptable. This includes Fall Rally and Rose Float Decorating, which are not considered overnight events. Overnight events incur overnight accommodations.

There must be one same sex chaperone for every ten same sex youth at overnight Key Club designated events. This refers to events that require overnight stays in hotel accommodations. Additional youth after ten requires an additional chaperone. THIS DOES NOT INCLUDE KEYLEADER. The Site Coordinator only has a specific number of slots for overnight chaperones, and accordingly puts same sex chaperones in cabins to oversee the students. 

As Region Advisor, I don't count as a chaperone at events, as I typically have specific other duties with the Lieutenant Governors. Therefore, each club is responsible for delegating and sending their own chaperones.

Driving students is also an area to be very mindful about. When driving students, it is best to remember the Rule of Three's:
Two adults in the car with one or more students, or at the very least, one adult and two students. In this situation, please make arrangements to drop off the last two students together at a final stop. NEVER drive 1:1 alone with a student."
